HOW TO CHECK YOUR INSURANCE BENEFITS

This page will guide you through checking your nutrition counseling benefits to ensure that our time together is covered by your insurance plan

Here at Heart N Soul Nutrition, we do accept insurance. We will bill your insurance for you after your session, however you are responsible to know and understand your benefits prior to meeting with us. See the steps and questions below that you can use when contacting your insurance company

If you are contracted with another insurance company we are more than happy to provide you with a superbill to submit to your insurance company for reimbursement for our sessions. The superbill does not guarantee reimbursement. *See self-pay discounted prices here. (We are not in-network with Medicaid/ Medicare/Disability)

Step 1

CALL MEMBER SERVICES

Call and ask:

1. Does my plan cover outpatient nutrition counseling: Medical Nutrition Therapy, weight management, or prevention?

See if these CPT codes are billable: • 97802 & 97803 • S9470

2. What conditions or diseases are covered (ICD-10 codes)?

• Common codes to use: Z71.3, E66.9, E66.3

IF COVERED ASK…

1. Are we in network?

2. How many sessions are allowed?

3. Do I have a deductible to meet first?
• If yes, how much is your deductible?
• How much of the deductible have you met?

4. Do I have a co-pay?
• If yes, how much is the co-pay?

*Often times with preventative counseling there is no out-of-pocket costs

Step 3

RECORD CALL INFO

Record the representative’s name and a reference # when checking your benefits. This information will be necessary if you ever need to dispute a rejected claim
